Unlabeled or Mislabeled Specimen Bottles

Pinkus Dermatopathology Laboratory promotes Laboratory Patient Safety Goals by maintaining high quality patient and sample identification at specimen collections, analysis, and resulting.

Our laboratory accreditations require our referring physicians and their offices to place at least two patient identifiers on every specimen bottle label. The most common and appropriate patient identifiers are: Name, Date Of Birth, Specimen Site, and the unique Patient ID Number.

The identifiers used must appear on both the specimen bottle and on the requisition form (biopsy sheet). Pinkus Dermatopathology Laboratory will contact your office if one or both identifiers are missing to resolve the nonconformance in order to process your specimen and not delay TAT.
